Diplomat Specialty Pharmacy Provides Chronic Kidney Disease Management For Pilot Programs

Flint, MI (Corporate Offices) September 6, 2006 – Diplomat Specialty Pharmacy announced today that their Chronic Kidney Disease Management Division (CKD Navigator) has entered into two pilot programs in Michigan. These programs include managing patients for McLaren Health Plan and West Shore Health Network (PHO), which at onset will impact over 400 patient lives. Both programs will last one year and will include disease management for Chronic Kidney Disease, Diabetes, and Hypertension. For patients who qualify, the program includes insurance prior authorization support, co-payment assistance, reminder calls for refills and free delivery. The CKD Navigator Program also provides clinical staff who consult with the patient’s primary care physician to assist in the care of the patient.
